Sweta Leung is an Associate Partner with Chartis, where she brings over 10 years of consulting and healthcare experience to the firm. She specializes in care model transformation, digitization of care, and consumer access and experience optimization, with a particular focus on healthcare at home and care at home strategy.
Sweta has worked extensively with provider and academic medical center organizations to develop and implement care at home and digital strategies. She has a proven track record of success in leading projects that help healthcare organizations transform their care delivery models to provide more efficient and effective care in patients' homes.
Most recently, Sweta has worked with a large academic medical center to develop and operationalize a comprehensive hospital at home strategy. She supported business case development, including care model refinement and a capability assessment to enable rapid scaling of the Acute hospital at home offering. Her work has helped the organization to achieve their strategic objectives of increasing access to care and improving patient experience.
Prior to joining Chartis, Sweta was a Senior Management Consultant at Accenture, where she worked closely with C-suite executives at large integrated health systems to develop strategic plans, operationalize patient and clinician-facing operational and digital solutions, and unlock financial value for the organizations.
Sweta completed her graduate and undergraduate schooling at the University of Minnesota, where she received her Master of Business Administration, Master of Health Administration, and a B.S. in Clinical Lab Science.
